A solution to home-based, digital blood pressure self-monitoring
Partner with Healthcentric Advisors, on a Special Innovation Project to reduce uncontrolled hypertension among Rhode Island residents and streamline care planning for clinicians. Our pilot automates provider access to real-time home blood pressure readings. The technology facilitates information sharing with providers between office visits, coupled with coordination of education and support services for patients and caregivers, will work to tackle hypertension in seniors.
