Healthcentric Advisors is going social in recognition of annual National Healthcare Decision Day and Centers for Medicare & Medicaid Services (CMS) 1st annual National Care Transitions Awareness (NCTA) Day, April 16, 2019.
During the week of 4/16/19 we’ll increase awareness of the importance of advance care planning and effective care transitions for our patient and provider communities.

- Engage patients in their own healthcare
- Help patients identify someone who can support them during care transitions
- Encourage individuals to consider what matters most – values/goals
- Support patients with documenting goals (advance care planning)
