The Rhode Island Department of Health (RIDOH) has awarded funding to Healthcentric Advisors to expand diabetes prevention and education in Providence through a partnership with Clínica Esperanza, a nonprofit health center providing free and reduced-cost care to thousands of Rhode Islanders.
Funded under RIDOH’s Community-Driven Outreach for Hypertension Management and Diabetes Prevention program, the project focused on increasing awareness of diabetes prevention and management among communities most affected by chronic disease and barriers to care.
Healthcentric Advisors developed a suite of bilingual (English and Spanish) educational materials to promote diabetes awareness, healthy living, and connection to local care resources. Designed with accessibility and cultural relevance in mind, these materials help patients better understand their health and take practical steps toward prevention and management.
To strengthen and sustain this work, the project also included a diabetes training for community health workers, equipping them with tools and knowledge to support patients in their own communities. These resources—created in collaboration with Clínica Esperanza—will continue to be used for education and outreach beyond the project period.
