High blood pressure, or hypertension, is a common, serious condition that affects about a third of U.S. adults. Proper management can decrease the incidence of stroke, heart attack, heart failure, as well as cognitive impairment and dementia in older adults.

Several challenges impact patients from seeking treatment, adopting a healthier lifestyle, or taking medication appropriately. Our Self-Measured Blood Pressure (SMBP) solution – combined with education, social support, and shared decision-making – helps engage patients in their care and empowers their caregivers and clinicians to create an appropriate care plan.

About our Pilot

The Centers for Medicare & Medicaid Services awarded Healthcentric Advisors the only Special Innovation Pilot in the country using our remote monitoring solution for uncontrolled hypertension. At the end of the pilot, 46% of recruited patients had successfully lowered their blood pressure into the desired range (<140/90 mm Hg).
