Increasing Home Health Face-to-Face Certification: Impact of a Hospital-Based Intervention

With support from our Rhode Island Healthcare Advisory Board, Healthcentric Advisors initiated collaboration between hospitals and home health agencies to improve patient transfers from the hospital to home. This collaboration led to interventions at hospitals in Rhode Island; most modified their discharge processes to embed required certification into the routine discharge paperwork for patients referred for home health services. Healthcentric Advisors’ initiated a study to examine whether the interventions increased completion of the required certification by hospital physicians who referred Medicare beneficiaries for home health services. Home health agencies in the state audited charts and collected data for the study. The team has published an article on the study in Home Health Care Management & Practice.
