Congratulations to Susan Dugan, the Director of Quality at Visiting Nurse Home & Hospice in Rhode Island, on her upcoming retirement!
Healthcentric Advisors would like to acknowledge her partnership with our organization over the past 15 years. She has been an outstanding model of a Community of Care Transitions Coalition Leader.
Additionally, she has always been a willing and valued partner in QIN-QIO quality improvement and care coordination initiatives. Including participation in our Home Health Collaborative, a leader in a project to improve the Face-to-Face CMS requirement on home health referrals, and a key participant in a project with URI College of Pharmacy that demonstrated the value of a pharmacist in-home health medication reconciliation.
These are just a sample of the many Healthcentric Advisors quality improvement projects Sue has spearheaded. After attending a Leadership and Organizing in Action training program offered by Healthcentric Advisors, Susan consistently demonstrated the tenets of that program in her role as a Coalition Leader.
We wish Susan the best in the next phase of her life journey – she will be truly missed!
