Advance Care Planning Event Round-Up

End-of-life conversations with loved ones can be some of the hardest we have. And because they’re so hard they often happen when it’s stressful or worse—too late. Yet communication with family members and healthcare providers about these preferences is a cornerstone of patient-directed end-of-life care.

To address this gap, Healthcentric Advisors, the Improving End of Life Care Coalition, VNA of Care New England, the RI Council of Churches, and Progreso Latino led a multifaceted, statewide project focused on improving consumer knowledge and engagement in advance care planning.

The education and tools within the My Care, My Choice, My Voice campaign empower people to:

(1) Think of what types of care they want;

(2) Choose their healthcare preferences, their healthcare decision maker, and document their wishes; and

(3) Voice their decisions to loved ones and healthcare providers.

In addition to spreading awareness about these resources online we’ve been holding interactive workshops in communities and physicians’ offices that offer advice and tools on how to have this important conversation.

As our project comes to a close, we want to celebrate and reflect on the amazing work done all over Rhode Island by our partners and give a roundup of events coming up:
