December Blood Pressure Pilot Data

Initial analysis of the data demonstrates that:

  • 54% of the patients participating in the pilot with uncontrolled BP (>140/90) have reduced their average BP to below 140/90 by the middle of January. This decrease from the previous month is related to multiple new patients being added to the portal in the last 1-2 weeks. Evidence to date shows >50% patients under control after approximately 4 weeks in the pilot.
  • To date, alerts have resulted in:
    • 32 medication changes
    • 68 calls to patients
    • 27 messages sent to patients via EHR portal
    • 3 office appointments scheduled

Be Recognized as a Champion!

Healthcentric Advisors will be recognizing the individual who has referred the highest number of patients with uncontrolled blood pressure to the SMBP Pilot that has resulted in active patient participation. This individual will receive a $150 gift card to Al Forno restaurant.

TIP: Our data indicates, providers that have had a brief personal conversation explaining the pilot to the patient and encouraging participation, have had the most engaged patients to date. Motivational Interviewing techniques may be useful tools to employ when discussing HTN control with your patients. View the Motivational Intervention Handout and Motivational Interviewing Strategies and Techniques.

The award will be given at the end of February. There is still time left to get your English and Spanish-speaking patients in the pilot!

We Heard You! New Portal Features

The portal ‘Panel’ feature allows you to create a customizable view of all of your current patients in the pilot.

Coming soon:

  • Sort by provider and practice site
  • View a dashboard highlighting patients who have not taken a blood pressure reading for > 72 and 96 hours
  • Document a note unrelated to a specific reading in the new ‘Notes’ section of the portal. e.g.: Call to the patient to encourage more frequent readings.

We hope these additional features make your experience more valuable.

Time is Running Out!

As a reminder, the BP monitors are distributed on a first come, first serve basis.

Be sure to get your patients referred before they run out!!
