Awarded Rhode Island State Innovation Model Grant “Increasing Patient Engagement in Advance Care Planning”

Providence, RI (November 13, 2017) — Healthcentric Advisors, a non-profit organization dedicated to advancing healthcare quality, was awarded a Rhode Island State Innovation Model Grant from The Executive Office of Health and Human Services to increase patient engagement in advance care planning conversations in healthcare.

My care, my choice, my voice

Active patient engagement in healthcare, especially advance care planning, is a cornerstone of patient-directed care. Patients should be encouraged to document and voice the care they want to receive. Unfortunately, these conversations often occur in stressful situations or when it is too late for patients to make decisions based on his or her values and preferences. Healthcare providers recognize the importance of and need for advance care planning conversations, however time constraints may impact their ability to initiate meaningful discussions with patients.

Healthcentric Advisors, in partnership with the Improving End-of-Life Care Coalition (a project of Rhode Island State Nurses Association), Care New England, Progreso Latino, and the RI Council of Churches, launched a multifaceted project to promote effective discussions between healthcare providers and their patients and families. This partnership will provide education and tools from the My Care, My Choice, My Voice campaign, developed by Healthcentric Advisors, to assist these conversations. This process empowers patients to (1) Think of what types of care they want; (2) Choose their healthcare preferences, their healthcare decision maker, and document their wishes; and (3) Voice their decisions to loved ones and healthcare providers.

In addition to targeted patient education within the Latino and Faith communities, the project also includes cross-setting provider education and outreach piloted in a group of primary care practices implementing group medical visits for Medicare beneficiaries and their families focused on providing information on advance care planning.

Ultimately, the goal of this grant is to normalize communication around advance care planning, ensure patient wishes are communicated, and provide a foundation for building a culture that embeds patient engagement and advance care planning in usual medical care.
