Solutions & Services
Over the past 20 years, serving as the Medicare QIO and private contractor for various providers and funders, we have built a robust team that can provide you with analytics support to make the most out of our data.
Data Analytics is more than just reports and charts. It is about understanding what to do with that information and turning into something useful and actionable. We can do that with you.

Transitions of care from one health care setting to another can often be a bumpy ride for the patient, caregivers and providers. Poorly-executed care transitions result in low patient satisfaction, high costs and sub-optimal results.
We have worked with providers to improve care transitions by implementing evidence-based interventions developed with input and feedback from stakeholders from all care settings.
We take our lessons learned and use it to help your organization optimize patient hand offs, enhance patient/resident and family education and provide staff education and training.
We will work with you to identify your opportunities for improvement and build a customized framework for improving care transitions.
