New England Healthcare Organizations Recognized for Excellence in Blood Pressure Control

We are proud to celebrate healthcare organizations across New England recognized with 2025 TARGET: BP awards from the American Heart Association and American Medical Association. These awards honor a strong commitment to improving blood pressure control and reducing the risk of heart disease and stroke, critical goals that closely align with our own focus on quality improvement and population health.

TARGET: BP is a national initiative grounded in five key pillars: Measure Accurately, Act Rapidly, Partner with Patients, Self-Measured Blood Pressure, and Equitable Health Outcomes. Awardees demonstrated commitment and leadership in implementing evidence-based strategies that deliver better outcomes for patients. New England is well represented among this year’s recipients, which include a range of organizations such as community health centers, primary care practices, and health systems, from across the region.

With nearly half of American adults living with high blood pressure, these recognitions underscore the power of structured, data-driven approaches to improving care. We are proud to support these efforts through our regional partnerships, technical assistance, and quality improvement efforts, helping providers put evidence into action and scale proven strategies across populations.
