Healthcentric Advisors is proud to acknowledge the recipients of the 2024 IPRO Quality Awards, specifically highlighting our nominated partners in the New England region. These awards honor healthcare providers who have significantly improved healthcare quality through their participation in the Quality Innovation Network-Quality Improvement Organization (QIN-QIO) program or the Hospital Quality Improvement Contractor (HQIC) program.


Healthcentric Advisors nominees include:

Overlook Nursing and Rehabilitation Center (Rhode Island)

Recognized for their outstanding efforts in care coordination, achieving American Heart Association certification for helping heart patients avoid rehospitalization. They achieved a 57% relative improvement rate for long- and short-term emergency department readmission rates. View poster.

Central Maine Health Care (Maine)

Central Maine Medical Center, Bridgton Hospital and Rumford Hospital
Honored for their exceptional work in reducing readmissions and improving patient safety. They reduced readmission rates from 18.0% to 14.8%, achieving a relative improvement rate of 18.1%, and maintained zero hospital-acquired pressure injuries for over two years. View poster.

Brattleboro Memorial Hospital (Vermont)

Nominated for their significant improvements in reducing readmissions, particularly for heart failure patients. They achieved a relative improvement of 23.3% in heart failure readmissions and a 30.1% improvement in COPD readmissions. View poster.

Athol Hospital (Massachusetts)

Recognized for their dedication to improving patient engagement and experience. They improved their CMS star rating from 2 stars in 2021 to 5 stars in 2024, with notable improvements in patient satisfaction scores across various departments. View poster.

Applewood Rehabilitation Center (New Hampshire)

Honored for their commitment to reducing preventable hospital admissions and readmissions. They achieved significant improvements in healthcare-acquired infections, ED visits, and maintained an all-cause 30-day readmission rate consistently below the state average. View poster.

Yale New Haven Hospital (Connecticut)

Recognized for their commitment to safe care transitions and cross-continuum collaboration. They implemented a data-driven, evidence-based approach to care coordination with a goal to reduce 30-day readmissions for patients discharged to skilled nursing facilities. View poster.

Valley Regional Hospital (New Hampshire)

Honored for reducing readmissions by focusing on care coordination. They reduced their readmission rate from 14.8% to 11.9%, achieving a relative improvement rate of 19.5%, and received a CMS Five-Star Rating in 2023. View poster.

Pemi-Baker Hospice & Home Health (New Hampshire)

Recognized for their ongoing commitment to improving transitions of care. They significantly reduced the rate of patients with an ED visit within 30 days of an inpatient stay from 46.7% to 10.5%, and reduced observation stays from 6.7% to 0% in the same period. View poster.

SASH (Support and Services at Home) (Vermont)

Honored for their success in chronic disease management, particularly their focused intervention to improve hypertension among older adults. Their Hypertension Management Intervention resulted in an average decrease of 7.5 mm Hg in systolic blood pressure among 718 participants over 18 months. View poster.

Ludlowe Center for Health and Rehabilitation (Connecticut)

Recognized for their organization-wide commitment to quality improvement and successful return to home and community. They developed the Passport™ program to deliver the best care and safely transition individuals throughout their healthcare journey, resulting in a 5-Star Overall rating from CMS and winning the McKnight’s Excellence in Technology Award for their partnership with Circadia Health.
