The Massachusetts Department of Public Health has awarded funding to Healthcentric Advisors to lead a statewide initiative to improve care for residents with substance use disorders (SUD) in long-term care (LTC) settings. Healthcentric Advisors, in collaboration with Alliant Health Solutions and with clinical guidance from Rossana Lau-Ng, MD, MBA, CMD, will engage in this 27-month project to equip LTC facilities with the knowledge, tools, and support needed to deliver compassionate, person-centered care. This initiative has been funded by the State Opioid Response (SOR) grant from the Substance Abuse and Mental Health Services Administration (SAMHSA).
The initiative will support nursing homes and rest homes across Massachusetts through:
· On-site training and tailored technical assistance
· Targeted quarterly educational sessions
· Annual community forums in eight regions to promote cross-continuum collaboration
· Curated best practice resources, redesigned for practical use
· A unique storytelling partnership with StoryCorps to capture and elevate the voices of residents and families
