Practice Transformation

Meaningful Use, Patient Centered Medical Home, PQRS, EHRs…those are just of few of the many drivers for Practice Transformation. We can help you prepare for successful practice transformation to meet your goals and avoid costly penalties.

We often begin by evaluating a practices readiness for change. From there we can develop a customized plan that meets your needs and your timeline.

Safe Transitions

Transitions of care from one health care setting to another can often be a bumpy ride for the patient, caregivers and providers. Poorly-executed care transitions result in low patient satisfaction, high costs and sub-optimal results.

For the past 8 years we have worked with providers to improve care transitions by implementing evidence-based interventions developed with input and feedback from stakeholders from all care settings.

We take our lessons learned and use it to help your organization optimize patient hand offs, enhance patient/resident and family education and provide staff education and training.

We will work with you to identify your opportunities for improvement and build a customized framework for improving care transitions.
